In general, it is not required to have a doctor’s note to receive a massage therapy treatment. However, some insurance company may require a doctor’s note prior to direct billing. Please check with your insurance if you need one. Doctor’s note is only valid for a year.
Massage Therapist Preparation:
– Will arrived approximately 15minutes before start time to set up the equipment.
– Will bring professional massage table with table warmer, fresh laundered linens, towels, blanket and premium massage oil or lotion. I can accommodate if you want me to use your own bed linens, towels, blanket.
– Ensure the equipment is cleaned and disinfected before and after each treatment.
– At the end of your session, will discard all used linens, towels, and blanket in a sealed plastic bag.
– Remove all – and store in a safe place to be able to massage the body area thoroughly.
– Please keep your pet and children away the treatment area and keep the floor area clutter free to be able to move around during your MT session.
Before your MT session, you will fill up the Massage Therapy Intake form and Consent and Authorization for Electronic Claim Submission and Assignment of Benefit form for direct billing.
We will update your file once a year or if there are any changes in your overall health condition. Additional assessment, treatment consent form will be discussed to you prior to your session as per necessary. All information is kept completely confidential.
I will perform an assessment and create a treatment plan that is safe and appropriate for you. I will incorporate combination of techniques and modalities such as myofascial release, trigger point therapy, swedish or deep tissue therapy, Joint Mobilization, PNF Stretching and incorporating various therapeutic modalities like medical acupuncture, hot stone therapy, reflexology, and cupping therapy. Will recommend some remedial exercises as part of your treatment and home care.
Yes, your extended health benefit may cover all or a portion of the cost of your massage therapy service and you just need to pay the remaining balance not covered by your insurance provider.
Please Keep in mind some insurance companies may require a doctor’s note. Check your insurance coverage prior to booking an appointment. If direct billing does not work, a full payment of the service is your responsibility to pay.
Payment mode accepted are Interac e-transfer, debit, credit card like Visa, Master card associated with your account is required to book an appointment over the phone or online. Your card will only be charge after your massage session. The card will not be charged unless you do not adhere to Cancellation/Rescheduling policy.
Your appointment is reserved just for you. Please provide at least 24 hours if you are unable to keep your appointment to accommodate other clients on the waiting list. If you are cancelling your appointment less than 24 hours, we will charge your credit card on file for 100% of the total treatment cost.
Current service area is City of Toronto (East York, Etobioke, North York, Scarborough).
Outside GTA, service fees is based on your home location.
Travel distance, traffic, time of the day, and weather are taken into consideration.
Outside 20KM require two to three people to book an appointment. Extra charge is only applied once at one destination per home location.
Please ask for estimate when booking an appointment by providing your complete home address.
Within 20km of postal code M3A 2Y1 is at no extra charge
21 – 35 KM away – $35.00 (require 2 people to book an appointment)
36 – 45KM away – $50.00 (require 2 people to book an appointment)
46 – 60 KM away – $65.00 (require 3 people to book an appointment)
Yes, for easy access please provide detailed instructions on how to get to a free visitor parking area around your location. For pay parking area a copy of parking fee ticket will be sent to you thru text message. Please note, If no parking space available in your area, No appointment will be accepted.
A valid credit card like Visa, Master card, American Express or debit associated with your account is required to book an appointment at the time of booking over the phone or online. The Organization will prepay upon booking and receive a single e-receipt, or each recipient can either pay individually after massage session.
No physical card will be issuing a gift certificate emailed to the recipient.
You can purchase e-gift card/certificate for any amount and send to multiple recipients.
All e-gift certificate value amount does not expire.
The e-gift certificate can not be redeemed for cash or refund of payment.
Gift Certificates may also be purchased in person, over the phone by calling or text message, payment can de done by credit cards.
Payment for e-gift certificate can be completed with credit card associated to sender account. Acceptable credit cards are Visa. Master cards or American Express.
The e-gift certificate may cover all or a portion of the massage therapy treatment and the recipient will be paying for the remaining outstanding balance. This can be use with any massage therapy treatment or spa package.
